Required Documents from the Nurse upon filling with USCIS.

1. Proof of passing the NCLEX. Can be the NCLEX-RN candidate Report.
2. Diploma for Nursing Degree
3. PCR license if licensed in the Philippines
4. Certificate/letters of employment
5. Birth Certificate from the Philippines Statistics Authority (PSA)
6. Marriage Certificate of spouse (PSA copy) if married
7. Birth Certificate of minor child/ren. Child is below 21 and unmarried.
   
   

Required documents from the Nurse at NVC (National Visa Screening)

1. NBI clearance if nurse is Filipino
2. Police clearance from any foreign country where nurse has lived for about 6 months after the age of 16
3. Passport
4. Birth certificate of spouse, if married
5. Annulment/divorce paper if applicable
6. NBI clearance of spouse if Filipino
7. Police clearance from any foreign country where spouse has lived for about 6 months after the age of 16
8. Passport of spouse
9. Passport of child/ren
10. NBI clearance of each Filipino child aged 16-21
11. Police clearance from any foreign country where child has lived for about 6 months after the aged of16 if applicable

1. Visa Screen Certificate
2. Updated employment contract between the petitioner and nurse
